A mite‑borne infection, scrub typhus is endemic in India. It causes a myriad of complications with multisystemic involvement like meningoencephalitis, acute respiratory distress syndrome (ARDS), myocarditis and also known to cause abdominal hepatomegaly, gastrointestinal haemorrhage, pancreatitis, renal failure. Among the organs, splenic considered be very rare usual presentation such cases splenomegaly. Splenic infarcts are lesser complication which has been sparsely reported literature. We report case 39-year-old male who our hospital fever system related complaints was ultimately diagnosed have multiple secondary typhus.
